At Good Grace Hospice, Inc. we pride ourselves on the personalized care and support we offer to each patient and their families.
Hospice Care Partners is a locally owned and operated hospice care provider in the Los Angeles Area. Our Hospice was originally formed in 2019 in Glendale, California with the vision of providing outstanding hospice care to our local communities.
We are now more centrally located to be even better able to serve the entire Los Angeles community. We are very proud to be accredited by The Joint Commission for excellent standards of practice for Home Care and Hospice.

What We Do
Good Grace Hospice is dedicated to protecting our patients and providing uninterrupted hospice care during the COVID-19 outbreak. We are available 24/7 including nights and weekends, wherever our patients call home (hospitals, skilled nursing facilities, assisted living facilities and private homes). We have implemented numerous efforts to ensure continuity of care and provide support to our patients and their families.
